Cove Physical Therapy is an out-of-network provider offering high-quality, one-on-one physical therapy.
I am not in network with insurance but can give you a superbill receipt to submit to insurance for possible “out of network” reimbursement or deductibles.

I accept the following forms of payment: cash, check, HSA/FSA cards, or credit cards. Payment is due at time of service. There is a 3% processing fee added to all card transactions.
NOTE: I am not a Medicare Provider. This means you cannot receive any reimbursement from Medicare for my services.
Initial Evaluation - $300

A comprehensive pelvic floor and orthopedic evaluation to identify the root cause of your concerns. This includes a thorough medical history, full-body musculoskeletal assessment, posture and functional movement screen, and a pelvic floor and abdominal assessment. After review of findings with you, you will receive a personalized treatment plan with home exercises and initial treatment techniques depending on your needs.
Follow up visit - $250

Follow-up sessions focus on reassessing your progress and advancing your home exercise program. Each visit may include therapeutic exercises, stretches, neuromuscular reeducation, soft tissue techniques (such as cupping, myofascial release, and massage), and personalized education. Appointments are typically scheduled weekly or bi-weekly, based on your needs and goals.
